"During the first evacuation of floors 7 and 8 on the night of 17 September, for a fire alarm, no announcement was made over the PA system, or if it was we were well and truly outside. No floor wardens were obvious at fire stair wells or exits. However on the second alarm a couple of hours later an announcement was made to guests to clear the building. The fire doors from each floor which open into the emergency stairwell as a requirement but there is hardly enough width to the landing to avoid people coming down the stairs from being struck by the heavy door shoved open by those leaving the floor."
